We offer post-acute clinical care at home. Our nurses will come to your place and work closely with your hospital team to support and care for you.
Our post-acute care aims to:

Our Clinical Nursing care includes:
- Assistance with medication
- Management of incontinence
- Changing wound dressings
- Checking blood pressure
- Assistance with travelling to and from medical appointments
- Organising visits from allied healthcare professionals
- Assistance with other clinical needs advised by your doctor
